Claude Code 2.1.287 introduces Claude Mods (plugins can modify deeper behavior), a new opt-in "You Should Know" side-agent plugin that flags things you or Claude might miss, an `n:<text>` filter in the agents view, support for URL prompts from MCP servers on the 2025-11-25 protocol (e.g. sign-in flows), `prompt_text` added to OpenTelemetry events, plus dozens of fixes across MCP, hooks, SDK streaming, Bedrock/Vertex, and screen reader mode.
